Course, logistics and awards

The route uses the vast majority of Ohio Route 45 and has just under 6,000 ft of total climb. The course has long flat segments and about 70 miles of rolling hills. A 6 day time limit for the full distance requires an average of about 34 miles per day. The 100 mile option begins at Wellsville Nicholson Stadium and finishes at Walnut Beach; the 100 mile cutoff is 72 hours and participants must provide their own transportation to the start.

This is an open road event with no official aid stations. Crews are encouraged in portions with limited overnight resources but are not required. Runners may need to self-extract if unable to continue. Stores, diners, and shops are available along the course during daytime hours, and several towns with hotels are within a few minutes' walk of the route, including Austinburg, Warren, Salem, and Lisbon.

Finishers receive a large Buckeye 200 buckle and a unique finisher award. $20 of each paid entry will be donated to the Wellsville high school athletic department. A "500 Lifetime Miles" award is given for completing 500 official miles on the course; five people had earned it as of 2025. The men's course record is 53:56:59 (2023) and the women's is 58:08:36 (2022); breaking a course record earns $100 and free future entry.
